ESSENTIAL FUNCTIONS:
The Advanced Practice Registered Nurse (APRN) in Nephrology is a registered nurse prepared at the master’s or doctorate level as a Nurse Practitioner or Clinical Nurse Specialist. The (APN) will manage primary and secondary care conditions, and evaluate tertiary care conditions. Primary care conditions are defined as common acute, chronic stable, or health care maintenance conditions; secondary care conditions are unfamiliar, uncommon, unstable, or complex conditions; and tertiary care conditions are acute, life-threatening conditions such as respiratory and cardiac arrest.

RESPONSIBILITIES:
Is authorized to prescribe and dispense medication when granted authority in accordance with State licensure

Order therapies such as occupational, speech, and physical therapy, and psychological counselling
May obtain comprehensive health histories and perform complete physical examinations, develop differential diagnoses, develop and implement plans of care, evaluate and document client’s response to plan of care

Participate in research as requested
Promote cost effective delivery of care
Role-model, consult, and educate nursing personnel to improve nursing practice and thus improve patient outcomes

REQUIREMENTS:
Graduate of an accredited school of nursing
Graduate of an accredited Nurse Practitioner Program
Current Advanced Practice Registered Nurse (APRN) Licensure
Current provider card in Basic Life Support (BLS) and Advanced Cardiac Life Support

Clinic Coordinator

Job Description: Clinic Coordinator

JOB TITLE:            Clinical Coordinator

ESSENTIAL FUNCTIONS:

Clinical coordinators are responsible for organizing and overseeing the administrative aspects of health services and hospital departments. They work in hospitals, clinics, academic medical programs, and other patient care environments. Among their daily duties, a clinical coordinator may plan budgets, oversee personnel hiring and training, schedule staff, manage supplies and inventory, and create and evaluate long-term programs and strategy for a hospital department. They may also ensure compliance with legal and hospital accreditation standards, oversee staff training and career development programs, evaluate effectiveness of patient care, and review staff performance. In an academic program, clinical coordinators may be responsible for overseeing student progress.

RESPONSIBILITIES:

  • Coordinate the rounds of prescribers within the facility: including communicating with prescribers when new patients are admitted, coordinating patients with facility staff to ensure patients are ready when prescribers do rounds, communicating with rounding providers when issues/concerns arise regarding medications and other medical concerns.
  • Manage the Medication Room including: intake of medications from the pharmacy, narcotic counts (1-2 times daily) with a facility RN, disposal of medications and preparing the medication carts for the facility medication passes.
  • Ensure the orders made by prescribing providers are sent to and received back from the respective agency (i.e. lab, radiology, pharmacy orders).
  • Be responsible for the prior authorizations for any medications/procedures are completed.
  • Manage the intake and output of CLIA waved lab tests/samples that will be done within CrossRoads facility units (urine testing blood glucose testing) including: handling supplies and sometimes samples, stocking supplies, ensuring checks on lab tests are completed.
  • Work closely with crossroads facility RN’s and other facility team members to ensure a seamless flow of medication management, diagnostic testing, and prescriber rounding occurs.
  • Ensure adherence to departmental policies and procedures.
  • Ensure services are in compliance with professional standards, state and federal regulatory requirements.
  • Monitors the research staff in effective and efficient use of laboratory and safety measures.
  • Coordinate inventory, order processing and distribution of products and services.

REQUIREMENTS:

  • Minimum of Bachelor’s Degree in a Health or Human Services field of study
  • 3+ years previous experience
  • Medication Technician certification
  • First Aid Certified
  • Must have time management, leadership, and record keeping skills in order to balance the multiple factors involved in successful clinical operations and patient care. Also, must have strong interpersonal skills to work well with other staff and interact positively with clients.

Picks up tenant laundry according to established procedures.
Refers to the Housekeeping and Laundry Tasks list to ensure tenant needs and preferences
are met.
Relates to tenants in a courteous, understanding, and cooperative manner.
Cleans office areas and other common areas according to a regular schedule or as
requested by the Maintenance Supervisor
Monitors for changes in the status of tenants and reports such observations to Maintenance Supervisor

Reconfigures, installs, positions, and remounts modular offices and space (e.g. furniture, wall panels, work surfaces, storage bins, lighting, file cabinets, etc.) to accommodate user needs and maximize office space using various hand, power and specialty tools, dollies and hand trucks .
Prepares the surfaces and paints various structures and equipment (e.g. walls, refrigerators, evaporative coolers, floors, roofs, doors, restroom facilities, etc.) to preserve wood and metal parts from corrosion and maintain a safe, comfortable working environment using various painting equipment and related tools (e.g. sprayers, rollers, brushes, thinners, etc.).

Education: Two years of experience in a private or governmental agency providing mental health services to clients, or completion of major coursework from an accredited college or university required for an Associate’s Degree or higher in a mental health related field (such as social work, rehabilitation counselling, psychology, counselling, or pastoral counselling).
Training and experience: Unless otherwise indicated, one year of current experience within the last three years in a comparable job classification required.
Working knowledge of the principles and practices used in the care, treatment and rehabilitation of psychiatrically or developmentally disabled clients.
Maintenance of current Cardio Pulmonary Resuscitation (CPR) for HealthCare Workers or Basic Cardiac Life Support (BCLS) certification for HealthCare workers.
Educated on and compliant with HIPAA regulations; maintains strict confidentiality of client information.
Complies with Infection Control, Standard Precautions and OSHA standards for the healthcare professional.
Able to wear personal protective equipment (e.g., face mask, goggles, latex/non-latex gloves) as needed.

ESSENTIAL FUNCTIONS:
Provides diagnostic evaluations and subsequent brief focused treatment using a variety of evidenced-based treatment interventions and various treatment modalities including individual, family, marital, and group psychotherapies.

RESPONSIBILITIES:
Oversees Mental Health Professionals and Mental Health Techs.
Manages all scheduling for reporting positions.
Evaluates and provides therapeutic intervention for patients requesting mental health services utilizing short-term solution oriented individual, marital, family, and/or group psychotherapy, and/or biofeedback, using verbal communication and testing skills.
Communicates effectively with outside agencies regarding treatment plans for referring agencies and/or outside providers; this may include preparation of written clinical reports and/or consultations.
Provides crisis intervention services as needed; determines appropriateness of hospitalization; assesses patient’s potential for lethality.
Must be able to sit majority of day during patient evaluation and psychotherapy sessions.

REQUIREMENTS:
Experience – Minimum two (2) years of post masters practice experience as a Licensed Clinical Social Worker, Licensed Professional Counselor or Licensed Marriage and Family Therapist.
Education – Masters degree in social work, counseling, psychology or an equivalent Master’s degree.
License, Certification, Registration – Nevada licensure as a Licensed clinical Social Worker, Licensed Professional Counselor or Licensed Marriage and Family Therapist in the State of Nevada.
For in-patient assignments – applicants must possess qualifications to be credentialed at inpatient facility.

JOB TITLE: Triage / Front Desk Coordinator

ESSENTIAL FUNCTIONS:
Coordinates all functions in the coordination of patient services, while in patients present in Triage. Work to complete insurance verification and electronic health record registration.

RESPONSIBILITIES:
Front Desk:
Check-in/check-out, answer incoming phone calls and enter information into the electronic health record.
Register patients into the electronic health record system and verify demographic and insurance information.
Answer phones and direct calls accordingly.
Obtain faxed data and routes appropriately.
All other duties as assigned.
Triage Desk:
Coordinate patients moving through CrossRoads continuum of care.
Coordinate patient interaction with RNs, LPNs, MHTs, MHPs, LCSWs and medical and psychological services.
Answer phones and direct calls accordingly.
Assist in patient care, as necessary to ensure standards of care.
Track, inventory, stock, organize, direct services supplies.
Track, inventory, stock, organize, office supplies.
Track clients through CrossRoads continuum of care, while in triage, using the electronic health record system.
